East Haven Nursing and Rehabilitation Center - SUMMARY: Assesses plans and implements the nursing care of residents with the skilled nursing setting with the supervision of a Registered Nurse. Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by CNAs and supportive staff members. Participates in resident and family teaching. Maintains role as resident advocate with a focus on the facility's mission. QUALIFICATIONS: Currently licensed to practice as a practical nurse according to Article 139, Section 6902 of the Education Department of the State of New York Demonstrates potential for competence as a nursing practitioner. Under standards scope of practice as a practical nurse WORKING CONDITIONS: * Works on nursing unit as well as throughout the facility * Sits, stands, bends, lifts and moves intermittently during working hours. * Is subject to frequent interruptions. * Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances. * Is subject to agitated and emotionally upset residents, family members, etc. * Communicates with other department supervisors. * May be asked to work beyond normal working hours occasionally. * Attends and participates in in-service educational programs. * Is subject to fails, bums from equipment, odors, etc throughout the workday. * May have exposed to infectious waste, diseases, conditions, etc... including the .AIDS and Hepatitis 3 viruses. * Maintains a liaison with other department supervisor to adequately plan for resident activities. SPECIFIC REQUIREMENTS: • Must be able to read, write and understand the English language fluently. • Must possess the ability to make independent decisions when circumstances warrant such action. • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public. • Must possess leadership ability and willingness to work harmoniously with and supervise professional and non-professional personnel. • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to relate to residents based on the level at which they are currently functioning. • Must possess the ability to seek out new methods and principles and be, willing to incorporate them into' existing practices. PHYSICAL AND SENSORY REQUIREMENTS: (With or without aid of mechanical devices) Must be able to move intermittently throughout the workday. Must be able to cope with the mental and emotional stress of the position. Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. Must be in good general health and demonstrate emotional stability. Must be able to lift push, pull and move a minimum of 50 pounds. Must be able to assist in an emergency situation; the evacuation of residents. Must be able to relate to and work with ill,' disabled, elderly, emotionally upset and hostile people within the facility. REGISTERED NURSE SUPERVISOR/PATIENT CARE COORDINATOR MUST BE NOTIFIED IMMEDIALTY FOR THE FOLLOWING: When a resident expires. Transferring of resident. Elevated temperatures, diarrhea, and untoward reaction to medication. Unusual procedures being done on the unit. When a physician is on the unit. STAT drugs. If any occasion arises that necessitates your leaving the unit. Chest pain of any type. Headache accompanied with blurred vision or nausea. SOB or dysphea. Pulse rate less than 60 or more than 100 or irregular pulse rate. When digoxin is held. When previous shift does not sign medications out. STAT medications to be ordered. When resident refuses medication or treatment. When resident refuses bath or shower for more than one week. Weight fluctuations as per facility weight policy. Any accidents/incidents. A discrepancy in the tally of controlled drugs. The RN supervisor/Patient care coordinator is to be notified immediately of any discrepancy in controlled drug inventory. No individual investigation is to be made. Any problems on the unit; i.e. fighting, resident disturbing other, non-adjustment, etc Difficulties of any kind involving residents, visitors and or employees. Complaints from residents, families and/or visitors. Assure and maintain a safe, stress-free environment for each resident.
