DescriptionThe Referral Care Coordinator serves as a vital bridge between the facility, marketing, and its clientele, as well as referral sources. With a dual focus on both nurturing client relationships and ensuring a smooth, continuous flow of referral communication and follow-up.Responsibilities: Develop and maintain relationships with the existing referral sources, such as hospitals, medical professionals, therapists, and community organizations for the purpose of streamlining patient care.Responsible for ensuring that coordination of care between clients and referral sources are maintained as deemed appropriate by client.Act as the point of contact for referrers as it pertains to patient care.Coordinate and conduct regular check-ins with referral sources, for patient updates, addressing any concerns and gathering feedback. Maintain strong rapport with counselors and clientele.Follow up with client's post-treatment to gather feedback, testimonials, and success stories and to ensure successful transition into their next level of care.Coordinate with the facility's care and administrative teams to implement suggested improvements when applicable.Collaborate with the marketing team to share, develop, and implement strategies and improved systems monthly.Maintain accurate and updated records of all client and referral source interactions in the facility's EMR, CRM and tracking systems.Attending discharge meetings to coordinate with referral sources.Assist with discharge planning and client discharge plan completion as needed.Act as a point of contact for coordinating Criminal Justice Legal correspondence for the program.Responsible for setting up Provider Referral Partners In-services for staff and patients.RequirementsREQUIRED SKILLSABILITIES: Excellent interpersonal and communication skills, with the ability to interact compassionately with individuals battling substance issues. Proficient in EMR, CRM software and Microsoft Office Suite. Strong organizational and multitasking abilities. Strong ability to manage multiple relationships with various agencies. Ability to maintain confidentiality and act with discretion and empathy.EDUCATION AND EXPERIENCE: Bachelor's degree preferred. Experience in client relations, case management, marketing, or outreach, preferably within a healthcare or substance facility setting is preferred.
