Job Location: Tulare - 1384 E Tulare Ave - Tulare, CAPosition Type: Full TimeEducation Level: Graduate DegreeSalary Range: $26.44 - $26.44 HourlyJob Category: Nonprofit - Social ServicesPOSITION SUMMARY:Person in this position will be primarily responsible for working with VAMCs and other healthcare systems ensuring appropriate care coordination for Veterans. Essential Job Functions include those listed below.ESSENTIAL JOB FUNCTIONS:Assist Veterans in getting access to appointments when needed.Assist with enrollment.Help gain access to appointments.Support Veterans in identifying health care needs.Work collaboratively with health care teams to facilitate access to care.Communicate with Veterans and health care teams about appointments.Assist Veterans in utilizing available services including preventative health care.Help Veterans identify barriers to adhering to recommended health care plans.Assist Veterans in understanding and communicating with providers to make informed decisions about health care.Problem-solve barriers to care (i.e., transportation, childcare).Provide education or create linkages for Veterans to learn about wellness-related topics.Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation.Perform all other duties as assigned.ESSENTIAL QUALIFICATIONS:To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.Ability to establish relationships with Veterans, health care teams, and community service providers.Ability to work under stress and in emotionally charged settings.Ability to exercise good judgment and discretion.Ability to work well in a team environment.Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality.Professional appearance and demeanor.Must be culturally/linguistically sensitive to populations served.Military/Veteran preferred.Must be able to successfully obtain and maintain a criminal background clearance.Valid driver's license and ability to obtain and maintain WestCare's vehicle insurance is required.CPR Certification, First Aid Certification, and an annual Tuberculosis test.During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.EDUCATION and/or EXPERIENCE:Master's Degree in Addiction Counseling, Criminal Justice, Social Work, Psychology, or a related field is preferred.LICENSURE/CERTIFICATION:Not applicable for this position.WORKING CONDITIONS:Work is primarily performed in an office setting.Some outdoor activities may be required.Work varied shifts, including weekends and holidays. #J-18808-Ljbffr
