JOB SUMMARY: The Referral Coordinator position works within the Texas Child Health Access Through Telemedicine program (TCHATT). TCHATT is funded through the Texas Child Mental Health Care Consortium and focuses on providing mental health support to students within the school setting by means of Telehealth. This position is responsible for managing incoming and outgoing referrals, engaging patient and parent involvement, coordinating meetings, data management and various administrative activities. This position provides administrative support and coordination for the TCHATT program to maintain a high level of efficiency. The position requires the ability to function effectively as a team member while also having the ability to work independently with moderate supervision. MAJOR RESPONSIBILITIES / DUTIES / CRITICAL TASKS: Facilitates, coordinates, and manages student enrollments for the TCHATT Program and works closely with the staff. Assists with preparing and submitting program documents including, but not limited to, consent forms, release of information, availability requests. Review documentation indicating who is legally authorized to sign for a child (i.e. custody documents, medical authorization forms, etc.) Inputting patient data (name, date of birth, insurance, etc.) into the EMR Document all correspondence, patient information, call attempts, and any relevant information on a patient in the Microsoft database system, and ensure all information is also updated with the state metric online platform. Engage school liaisons when needed to improve relations and engagement of parents. Assists in maintaining and monitoring program level data. Management of outgoing referrals including contacting organizations and providers, attempting to ensure compatibility, relaying patient information, and establishing patient with referral source. Serves as liaison to the TCHATT program and provides support and information when needed regarding policies and procedures. Demonstrates professional behavior in all interactions and utilizes proper telephone etiquette during every call. When needed, performs patient appointment scheduling, registration, insurance verification, check-in, and check-out procedures with the goal of 100% accuracy. Adheres to internal controls established for the department Participate in community outreach activities to raise awareness as needed. Perform other related duties incidental to the work described, such as attending trainings and regular team meetings. Handle confidential and/or sensitive information with discretion and adherence to HIPAA rules Other duties as assigned. ACCOMPANYING KNOWLEDGE, SKILLS, ABILITIES AND COMPETENCIES: Demonstrated knowledge working with deadlines and under pressure and the subsequent ability to prioritize tasks. Ability to expand on mental health services that are provided and the different levels of professionals associated with the program. Ability to be flexible, make sound decisions and demonstrate problem-solving skills. Excellent written and verbal communication. Extensive experience with the use of Microsoft Office Products, Adobe Acrobat Pro software, and an ability to learn various other programs (ex. SmartSheets or DocuSign). Demonstrates professional behavior with sound work habits. Customer service skills. Ability to multi-task in a fast-paced working environment.
